Topic: Spread of Odontogenic Infection:
INTRODUCTION:
Odontogenic Infection could originate from the following foci as an abscess

  • periapical periodontis- Dentoalveolar abscesss
  • Periodontal infection- Periodontal abscess
  • Pericoronitis around an impacted tooth- Pericoronal abscess

From these places the abscess could spread as follows:
  • Cervical Lymphadenopathy
  • Cellulitis-Involvement of Deep Fascial Spaces
  • Osteomyelitis

Cellulitis-Involvement of Deep Fascial Spaces

First these primary areas can get involved in the spread of odontogenic infection
From upper anterior teeth
     Labial space, Palatal space, Canine Fossa
From upper posterior teeth
   Buccal space, Palatal space, Maxillary sinus          
From lower anterior teeth
Labial space, Sublingual space, Submental space
From lower posterior teeth
Buccal space, Sublingual space, Submandibular space
These Secondary  Tissue Spaces  can get involved with persistent infection
Submasseteric space, Infratenporal space
Other Deep spaces- Lateral Pharyngeal, Paraphrayngeal, Retropharyngeal, Mediastinal

Involvement of multiple spaces:
Ludwig's Angina
Bilateral involvement of Submandibular and Sublingual spaces

Principles of Management
Abscess and Cellulitis
Removal of the focus if infection
Incision and drainage
Antibiotic therapy

Submasseteric space Infection- through-&-through drainage
Ludwig’s Angina
emergency admission
IV antibiotics
Airway management
Multiple incisions to relive pressure build-up in the neck
